Perhaps it's time for a new fan. When they begin getting worn out they become rather rattly sounding and less efficient. Look around on sites like newegg. If the fan on the heatsink is the noisy one, it's pretty easy to replace the fan or simply replace the whole heatsink. Just be sure you get the correct heatsink for your processor.

dust inside can cause extra noise, and as the_other_ one says perhaps it's time for a new fan

74 degrees is very very high, you have some serious overheating issues and should take the time to understand why, it is highly likely that due to the overheating, the fan are on the highest speed they can go which would contribute to the noise.

Be a good idea to clean that thing out, if its that dusty I would say the CPU heatsink fin are cloged up with dust. I would clean the heatsink and fan up and see what happens, if its still making noise get a new fan or combo.

StrangleHold

yeah you should should check if the sound is coming from the case or the cpu fan first. and stranglehold is slightly right. the dust could be clogging the circulation of the fan up. but that doesn't happen too often.

so if the sound is form the case you can buy some antivibration accessories.

I've had this computer for around 4-5 years. Haven't cleaned it out once. The amount of dust was about half an inch. Once cleaned out, the case felt cooler, less noise and computer is going a bit faster. I have a P4 1.70GHZ. Very slow hence I'm going to build a new computer.

You should clean your case out every 6 months or so, depending on where you live and how dusty your area is. Dust causes heat build up fairly fast.
